The tirzepatide ladder, and what a five-day half-life means

Tirzepatide is dosed once weekly and steps upward in 2.5 mg increments spaced about four weeks apart, which is a longer climb than most home-injected protocols. Its half-life of roughly five days means it reaches a steady pattern in about four weeks on a given step — so each rung has just about settled by the time the next one arrives.

The FDA labels describe a ladder from 2.5 mg weekly up to a maintenance dose of as much as 15 mg weekly, in 2.5 mg increments roughly four weeks apart. How far up it you go, and how fast, is set by your provider.

Its published half-life is about five days, so most of a dose has cleared within a few weeks of stopping. That is also why the curve between weekly shots dips a little more than semaglutide's does.
